Pre – Interview In this phase of the process, a new patient has just arrived for their appointment with the Physiotherapist. They are greeted by our friendly Admin team who will then ask them to fill out the intake form. The intake form is very similar to the interview or the subjective assessment. The form allows the new patient to share as much detail as they wish regarding their symptoms and what they require assistance with by answering a series of questions.
