First Visit Physio Parramatta

On your first visit we try and understand your problem in great detail, discuss your needs with you and begin a hands on treatment process. Our Physiotherapists are up-to-date with current research on injury management. We strive to make our patients’ journey as comfortable and stress-free as possible.

Please arrive 5-10 minutes before your first appointment to complete a new patient form.

What to bring to your first Physiotherapy visit

All Patients

• Any doctor’s referral or letters
• Relevant post-operation report
• Relevant imaging scans (eg X-tray, MRI, CT, Ultrasound)
• A current list of your medications

Worker’s Compensation Patients

• Name of insurer
• Claim number
• Letter or email from insurer indicating that liability has been accepted
• Employer detailsWe also accept DVA and NDIS clients.

Referral Letter

A referral letter is helpful so we have some background information on your injury or condition. We are always happy to discuss via phone or converse by email if you have any questions.

Shorts for lower limb injuries and singlet tops for neck and shoulder injuries. We can provide shorts and gowns if needed.

• Worker’s compensation, NDIS and Veteran’s Affairs clients – please bring all the required information
